Applicant
must fill up the necessary application, together with the complete
basic requirements and index card, then file it with the DTI Cebu
Provincial Office.
Processing/classification
of filed application. The following requirements must be attached
to the application.
For
Original Application:
- Copy
of comprehensive insurance policy. Insurance covers the customers’
properties entrusted for the repair/service and copy of official
receipt covering full payment of premium or undertaking re-exemption
or affidavit on jobsite
- Warranty
or service guarantee duly signed by the owner of the shop
- List
of shop employees and position
- List
of shop tools and equipment and value
- TESDA
(NMYC) Certification of chief mechanic/technician
- Size
of shop/number of stalls
- Articles
of Partnership or Incorporation
- Shop
Inspection
Additional
Requirements for Three to Five Star:
- Photos
of shop – showing front and inside which includes facilities or
major installed facilities and/or equipment
- Shop
layout with dimensions
